Job Description
Ensure the safety, comfort and well-being of the clients
Work with the therapists on implementation of new activities
Work with nurses to maintain a safe and clean environment
Participate in quality improvement committee, and serve as a role model
Conduct home visits to residents in the community as part of the community outreach inititatives
Work closely with Community Outreach team and MSW to follow up and coordinte the care needs of community residents.
Provide support and involve in the planning and coordination of Active Ageing Programmes.
